The Bruce protocol is a maximal exercise test where the athlete works to complete exhaustion as the treadmill speed and incline is increased every three minutes. The length of time on the treadmill is the test score and can be used to estimate the VO2 max value.The Bruce Protocol test is one of the simpler methods of estimating VO2 max.
During the Bruce Protocol Test, you will be working to exhaustion to see how long you can last on the treadmill as your speed and incline will increase every three minutes. Each stage below represents a three-minute interval. Stage 1 = 1.7 mph at 10% Grade.Bruce Protocol Stress Test The Bruce Test is commonly used treadmill exercise stress test. It was developed as a clinical test to evaluate patients with suspected coronary heart disease, though it can also be used to estimate cardiovascular fitness.
See also a general description of Exercise Stress Tests.The bruce protocol stress test or the Bruce Treadmill test protocol was first designed in 1963, by Robert A. Bruce. Bruce protocol stress test was a non-invasive stress test to assess patients with probable or suspected weak heart conditions.
Other than a stress test it is also called as the exercise tolerance test.According to the original Bruce protocol, the patient walks on an uphill treadmill in a graded exercise test with electrodes on the chest to monitor the EKG. Every 3 minutes, the speed and incline of the treadmill are increased. There are 7 such stages and only very fit athletes can complete all 7.Following the warm up period, the Bruce Treadmill Test actually starts.
To perform the test, an individual should start jogging on a treadmill set to a pace of 1.7mph and an incline of 10%. This is the first stage of the test and should be kept up for a total of 3 minutes. Every following stage of the test should also be kept for 3 minutes.
A Bruce exercise test involved walking on a treadmill while the heart was monitored by an electrocardiograph with various electrodes attached to the body. Ventilation volumes and respiratory gas exchanges were also monitored, before, during and after exercise.THE BRUCE SUBMAXIMAL TREADMILL TEST IS PERHAPS THE MOST common test used to assess cardiorespiratory fitness, especially in clinical settings. The test is administered in three-minute stages until the client achieves 85% of his or her age-predicted maximum heart rate (MHR).
The Bruce protocol is a standard test in cardiology and is comprised of multiple exercise stages of three minutes each. At each stage, the gradient and speed of the treadmill are elevated to increase work output, called METS. Stage 1 of the Bruce protocol is performed at 1.7 miles per hour and a 10% gradient.
An inability to exercise >6 minutes on the Bruce protocol, or an inability to increase heart rate (HR) to >85% of maximum predicted heart rate (MPHR) are significant indicators of increased risk of coronary events with a 5-year survival ranging from 50% to 72%.It is worth emphasizing that in the most heavily trained athletes, a standard Bruce graded exercise protocol may not be sufficient and other more aggressive exercise studies may be needed. 27 More aggressive graded exercise protocols such as the Astrand and Astrand-Saltin are done at increased speed and incline to help accommodate the greater aerobic capacity of an athlete.Bruce test is considered a maximal exercise test where the athlete or client runs to complete fatigue as the treadmill speed and incline are increased every three minutes.
Record total running time.A variety of treadmill fitness and stress tests are available, but the commonly used treadmill ramp test is the Bruce protocol. It is the easiest to administer and is widely available in clinical and fitness locations. This is the amount of oxygen in milliliters your body can use per kilogram of body weight per minute.Participants were asked to complete two maximal treadmill running tests (using the Bruce and the Athlete-led protocol) to volitional failure in a randomised cross-over trial 1 week apart.
The participants were asked to refrain from intense exercise and alcohol for 24 h prior, and caffeine from 4 h prior to each main trial.The Bruce Protocol is a maximal exercise test where the athlete works to complete exhaustion as the treadmill speed and incline is increased every three minutes (See chart). Had a stress test 4-20-17 thought they was going to kill me wasn’t a walk in the park.
I have a question? So do you fail the test if you are unable to get to your max HR? My max HR is 191, but when I do my test I find that I can only get to about 175-180 BPM, before I tire, however once I quit the test I do recover within 60-90 sec’s.

what does the grade mean? the one you are referring to as percentage?
Is there a rule whether or not you are allowed to hold onto the rails? Also, does it make a difference whether you do or not? I recently had one at UC Davis and they did not let me use the hand rails saying that the moment I touch the rails, the test is over. at 8.5 METS I had to hold, requiring a 12 METS minumum for a law enforcement job. I thought it was unfair because I have done them before for another agency and I was allowed to hold on.

I am required to get a stress test after my open heart surgery, to return to work. How hard is 8.5 mets? Cardio rehab has been canceled due to covid 19. Ive seen 6.5 mets in rehab before. I`m 61
@gcot29 Are you familiar with the type of exercise you were doing? If it was a bike then your muscles may become fatigued before you reach your max cardiac output. Is your max HR predicted? did you do the 220-age formula? If so then that is just guess. If you do the stress test and it comes out to 180 then your Max HR is 180. As long as ur within 12 of your predicted HR it’s okay. AND you have to put MAX effort when you do the exercise test, suck it up if you want an accurate measurement.
I’m going to get mine tomorrow, and I’m going to nail it. I won’t last more than 2 3 mins. My heart rate will go over 185 in that time frame. They will be floored how fast my heart beat jumps. I can probably keep that up for a whopping 30 seconds when it gets to that level. Finally, someone is going to believe me that I’m not joking when I say I can’t run more than quarter of mile.

I just did mine. I had to achieve 100% of my maximum predictive heart rate. 165 bps. I’ve done 140-150 bps on a gym treadmill, but 165 was a whole different ballgame. I’m in decent shape but I’m no athlete…..and I’m 55. It was without a doubt the hardest physical exercise I’ve ever done. I understand most ppl just have to go to 85%….which I could do easily.

For AHA accurate BP readings require the arm be supported & cuff be level with atria, neither of which is demonstrated in this video. Student should be supporting his elbow so that he is not holding up is arm for her. Yes his hand is resting on her shoulder, but he’s keeping it from falling off, using isometric contraction around the brachial artery which elevates BP slightly. Because the upper arm is lower than atrial level, this also increases BP slightly. The patient/student position is better in this video (she is not pulling his arm behind him to obtain the BP as she did in the other video).

HRmax = 205.8 (age x 0.685) is a much more accurate formula than using the standard 220 age for predicting HR max…

I had this test, which was normal, has several pvc’s which doctor said was normal. However, he said I have a poor fitness level. On what criteria was this based, does anyone know? He didn’t explain further, other than I should exericse more. I bike at an aerobic level for 40 min to 1 hour 2x a week 6-10 miles each time, ballet 1-2 times a week and lift weights. What more am I supposed to do? How can I have a poor fitness level?
I took a stress test thought they were going to kill me couldn’t finish all the levels ended up with four blockages had open surgery next day.
I did this with Ease… I had one done today. I found it simple I didn’t even breath heavy at all after I was done it was like I didn’t work out at all didn’t even sweat but the guy in the video did good no doubt about it 18mins is very good the norm for most people is 8 mins
You scrubbed his flesh off if a patient saw those bruises, they will run away in the middle of the test at the speed of 5.5 MPH (just so that you may not be able to catch them)
Very helpful, but surely she should be using her fingers to actually locate where the electrodes go rather than just rough guessing? I know we’re always under time constraints but it seriously affects results, especially when exercise testing. Also there’s no need to leave such red marks on his skin when abrasing. It’s just unpleasant for the patient and not necessary.
I had a health scare in 2011 (spot on my lung that turned out to be a calcium deposit). I drank lots of water, ate healthier and exercised regularly by using the pool, walking and dumbbells at home. When I had my stress test, I was supposed to get to 140, but couldn’t get past 122. They were amazed by what I was doing to keep in shape (I was 55 then).
Fast froward to 2019..I’m having a nuclear test done on 12-5-19 due to my hip replacement back in January. Even if I could do the treadmill, the nuclear one would still be better for me since I’m so out of shape. Even though it’s 8 years later, I need to get back into my old habits.
Not a VO2 max test! A 12-lead EKG stress test is used to diagnose cardiac conduction problems. No mention of the function of the various leads. If you just need heart rate, use a simple heart rate monitor.

My understanding is that you alcohol first than exfoliate to minimize patient discomfort. Also there was no immediate BP taken when the patient stopped the test. Overall pretty good quick tutoral. Would have been nice to see all the steps leading up to the actual test i.e. heart axis, hypervent etc.
