Any good stories of someone who stole your thunder?
These are a few I had recently, not really anyone intentionally steeling my thunder but it sure felt stolen.
- Patient follows up from urgent care. He was seen for left lower quadrant pain and fevers the day before and gets diagnosed with UTI and started on Cipro. I see him and immediately think something is wrong. He has a fever of 102 and has LLQ guarding. Order emergency CT and he has diverticulitis with perforation. I send into the hospital with my completed notes and the H&P says "PCP treated for UTI and CT done in the ED showed perforated diverticulitis" GAH! I didnt miss the diagnosis, I'm the one that caught it!
- Seeing a patient for a cough after not improving after seeing another doctor. Has a new hypoxia and says her face feels swollen. Cant hear left sided breath sounds that well. I put r/o SVC syndrome in my assessment and send for urgent chest xray. Sure enough, 8cm hilar mass. I send in to the hospital. CT confirms mass is pressing on SVC. She gets transferred to larger facility and H&P says "pcp concerned that medication was causing allergic reaction on face, CT ordered by the ED confirming mass" GAH! I wrote up my note quickly so they had all the info and they still give the save to the ED!
