Under healthy conditions were not breathing, evaporating or urinating out our glucose or nonpolar lipids (although itd be interesting to see how SGLT 2 inhibitors or alpha glucosidase inhibitors change physiological markers)
Of course a physicists or physical chemist would claim “energy in = energy out” model due to conservation of energy etc. So of course it’s true, however hormones can change the efficiency of free energy lost from a system per unit time…
so if Rhonda and Peter each eat 2000 calories over a 24 hour period, but due to Peters hormones he loses more free energy per unit time, then he would lose more weight…
Of course all of these theories would all be superceded if we discovered some uncoupling effect due to keto metabolites… or thyroid effect of turnover and therefore more heat loss per unit time…

Ive heard Gary’s Taubes promote the “insulin promotes fatty acid synthesis and inhibits fatty acid oxidation” so therefore you gain more weight…. but if you eat a fatty meal… the energy in that fat is going to be stored in the body regardless… it’s not going to be urinated or breathed out… so if you eat carbs the energy in them will be stored in the body as fat… if you eat fat the energy will still be stored in the body as fat…
In a fed state you’re catabolizing carbs to anabolize fat…
In fasted state you’re catabolizing fat to anabolize carbs and ketones..
I don’t understand why one would be substantially more or less energetically efficient compared to the other…. if anything, when you eat carbs insulin causes biosynthesis of fat which is actually unfavorable because it’s anabolic and would require extra steps which are energetically inefficient and would lose weight… rather than eating carbs and by passing that inefficient biosynthesize of carbs…

Love this podcast. I use extended fasting to ameliorate sleep deprivation effects as a neurology resident. It is quite effective. Currently just finished a 4 day fast while on night float and will do a 36 hour fast when going back to days. Haven’t gotten sick In 2 years since starting cycling extending fasting despite a fairly toxic lifestyle with constant sleep deprivation and stress. Exercise is a must as well even when sleep deprived.

While doing the HCG diet and experiencing stalls. The trick i learned was to fast all day EXCEPT eat 12 to 15 Mac nuts throughout the day. the next day i lost over 4-5lbs. they released fat. there is truth that mac nuts are a miracle food.

I don’t recall where I got this fact, but T2 diabetics engage in gluconeogenesis 3x as efficiently as non-diabetics. Though I’ve managed to get my bG into normal range (65-90) after weaning exogenous insulin via Fung’s methods, I’m relatively sure metformin is necessary to this achievement. Hopefully, it won’t be necessary forever, but after 30+ years as a T2, I don’t know if my body will lower gluconeogenesis to a normal level. I wonder if that higher rate of gluconeogenesis is a partial cause of T2 in the first place; it certainly helps maintain elevated bG if your body “thinks” your homeostasis is 200 mg/dL and kicks in gluconeogenesis to maintain it. I don’t know how long it will take my liver to understand the “thermostat” has been turned down.
I’m not sure how one could measure gluconeogenesis, knowing the bG was from that vs glycogen or food intake, but I’d suspect your patients that have issues getting into ketosis have a higher rate than normal.

I’ve heard while on the Ketogenic diet, basal metabolic rate increases due to a mitochondrial uncoupling effect on adipose tissue… But I’ve also wondered… if you’re on a ketogenic diet, you would literally breath, evaporate (from skin) and urinate out energy from your body at an incredible rate…
For example breathing out acetone plus evaporating acetone from skin plus urinating BHB and other ketones would create an insanely sharp differential of chemical potential loss per unit time…
With that ham and those ribs and those wings… you should have a Fan Appreciation Cookout at your home for all of us!

I fast everyday from 18:00 hour til the next day 12:00.
Then I start with bulletproof coffee with MCT OIL
My hunger start round 16:00.
I start eating around 16:30
Sometimes eggs, bacon, cheese.
Salade with olive oil and apple vinegar.
Sometimes with 150 gr Greek yoghurt 10% fat.30 gr Walnuts, 50 gr blue Berries. From time to time extra cheese 48+.
We switch it of with brocoli soup, etc.
Is it OK or do you have tips for me.
