I have a patient who continues to have candidal infections over and over again (usually vaginal but also other places) with seemingly no "cause". This has happened for a year despite every treatment you can think of. I have worked them up extensively, gotten immune deficiency workups, rheumatology consults, infectious disease consults and allergy/immunology consults. They have been to the mayo clinic and everyone is pretty much stumped.
I randomly decided to pull an IL-17 on them based on its importance in candidiasis with the thought it might be abnormally low or zero (some specific targeted immune deficiency that causes only this recurrent infection).
Well, to my surprise, it came back absolutely enormous. 209 pg/ml
Should I expect this as the immune response to candida? Or is there something else here I'm overlooking? I really want to help this person, but I've sent them everywhere I can think of with no results and they just keep coming back to me expecting a solution. I want to give them one, but I'm grasping at straws at this point.
