This approach caters to the more engaged, more affluent consumer. There will be a niche market for such services and information. If we want to transform the entire society, we need to change policy so that anyone who puts pollution or fertilizer or antibiotics will be required to pay a fee (if most people feel that such actions should be discouraged). We can use random surveys to discern whether the overall amount of pollution or rate of taking of this or that resource is acceptable. By adjusting the fees, we will get the world that (the average opinion of) the people say(s) we should have. That would be a real democracy. We could give the proceeds of all fees collected to the people.
When overall rates of putting pollution and taking resources matches what the people think is most appropriate, because the right fees are being charged, the prices would reflect those impacts that some of us may not be interested in researching, but that most of us think are worthy of consideration. Prices would reflect true costs. (That price in SuperSaverMart would not be lying to us about the true costs… That which is harmful to the environment or to society (because it accelerates development of antibiotic resistance, for example) would have a higher price tag. The most harmful practices would be eliminated from industry and agriculture.

Few realize a SCOTUS decision of the past has brought about a change to society that none could foresee. Lawyers were allowed to advertise and advertise they do. Everybody is aware of the ads on television promoting greed over responsibility. Doctors became “deep pockets” and perfection was expected or a law suit would follow. Even today, as our Legislatures discuss “stimulus”, protection from vulture lawyers is a topic. SAD
The problem is that the medical field actually does not know what they are doing, like an electrician or plumber. They are “practicing” and this is a very telling difference.
I don’t trust doctors implicitly. I do not feel they have my best interests at heart, but rather the interests of the organization they are in.
One blatant example is routine circumcision of infants. It is done on healthy patients and results in the loss of a healthy functional part of the body of the patient. And it is done without ANY of the normal protocols concerning surgery. They are actually performing an amputation without any diagnosis and as such it is a violation of the ethical standards they should be following. Now you know why I really don’t trust doctors.
By many lawsuits they have become defensive. The enemy of this is us.
Unfortunately, the so called healthcare system in the US is fundamentally broken and in dire need of a complete overhaul (hint: single payer). And yet, even in the best healthcare systems worldwide, more transparency is still needed. Thank you Dr. Leana Wen for serving humanity nobly.

Good on her. The arrogance of these doctors what are they afraid of? I’m a surgeon and would be more than happy to sign up (if we had an equivalent system in Australia). Most of us became doctors to help patients not lie to them
Dr. Wen suffers from naivete or idealism; which are just aspects of the same thing. While there are patient’s and families that will feel comforted by your taking the time to inform them, there are patients and families who hate authority figures and are hateful and suspicious of institutions and will use any information you give them against you. Having tried for years to provide the most information to families and patients myself, I have the emotional scars… the cost of such naivete. There are even patients who will hate you for not going along with their insistence that there is something wrong when there is nothing objectively wrong, but they have some agenda which has nothing to do with their actual medical condition. This is really, really the speech of someone naive and idealistic… and I completely understand both what she is saying and the reaction of her colleagues. For some patients, not as rare as you might think, seeing a doctor is like playing a game show where they actually hope something will go wrong so they can sue! I wish all my interactions with patients had been taped for all the extensive time spent explaining things to people thoughtfully who turn around and insist you never mentioned that! It has become a sport among certain groups in certain places in the country that certain kinds of professionals are fair game. In certain places in this country, it can be exactly the opposite where everything you do is appreciated. I hope Dr. Wen finds the right place for her idealism.

Doctors shouldn’t be biased, but they are. So transparency is necessary. Because although we all doctors for services, we aren’t asking them to make life decisions for us, that we have to live with, in the end.
I’m so lucky, my Doctor sits down with me and asks how I’m doing every time I go in. He genuinely tries to help me. I’ve literally kept him as my doctor for that reason, even when i had to switch insurance companies or pay out of pocket. When i told him I couldn’t afford it after losing a job, he told me my visits would be free after that and he would figure out payment later. Even though it was for minor check ups, it made me feel like somebody cared. Because of that, I felt comfortable enough to go see him when my depression was at its worse. And he helped get find a proper psychiatrist and some support groups. Kudos to all the Doctors and Nurses out there right now putting their lives on the line to fight COVID19.
She’s fighting the symptoms, not the disease. Such practices will continue so long society can put a price tag on human life.
Wow…the doctors’ reactions to calls for transparency was very shocking. It confirms everything I suspected about them…they are conflicted humans just like everyone else. This is the main reason Google is my main health provider.

You are so right in New Zealand our public health sytem is so broken the doctors deciding your treatment are a panel that have never seen you in person and often ignore the doctor who did’s recomendations. And practice a tick box excercise. Talk about low trust we have zero trust in our doctors in NZ
My priority is preventing illness. I wish I could find a doctor who was only paid if I stay healthy. That and the great long waits for a doctor are ridiculous. If a doctor keeps me waiting I find a new doctor.

The only solution I can think of for the oncologist who has been bought off by the drug company is to ask and explain: ” I am fully aware of the relationship between doctors and drug companies. Is this really the best choice, or the one that makes you the most money? My survivors without my asking have decided to hold you physically responsible. And now that you know, it IS too late to back out. Wanna recheck my chart?”
If not law, maybe more transparency via a website or organization? Not objection by a hospital, one for all doctors from different hospitals.

I’ve had a stroke when I was 23. Upon this event they found my heart had a small hole, 3mm wide, between the chambers, that could have led to the stroke. My cardiologist said he could fix it. I trusted him, so I signed the papers for surgery. When I came home, I found out they’d put a 3cm device in my heart, versus the 3mm hole that was there before.
At 25, I had a shivering fever for 3 months, my doctor kept saying “it’s just a flu, take this herbal remedy”. But I kept on losing weight, and shivering, to the point I was 40kg (dressed), and the whole bed trembled when I was shivering. I called a virologist, and he immediately hospitalized me. It was endocarditis, a disease that could lead to death, if not treated with antibiotics, and it was located right underneath the 3 cm device.
But hold on, it’s not over.
At 27 I went to have my usual cardio-check. I read on the paper “severe atrial dilation” and I asked the cardiologist (a different one) what was the meaning. He said nothing, and went on with his patients. I guessed it was not important, maybe a side effect oh antibiotics. My life went on, while I was busy juggling between my 2 jobs, my breathlessness and my inexplicable incapacity of walking on a slope.
At age 30, at another cardio check, the cardiologist said “oh my, this is so severe we need to hospitalize you for further examination”. But I had to go through other two hospitals to get my diagnosis. Turns out it was PULMONARY HYPERTENSION, secondary to that endocarditis.
I was assigned to a doctor (one of the very few who’s had his specialization in PH) that every time I speak a symptom, or a thing I would like to do (I’m 32, I have a hunger that you will never taste), raises her voice and talks to me like I was a child, and who I never empathyzed with. In fact, when I had severe menstrual bleeding, due to the anticoagulant they were giving me, she said “there’s always hysterectomy”, removal of the uterus. By this time, I was confident enough to pack my bags, ask my fianceé to drive me to a town nearby, which had a gynecology hospital, and university. I had emoglibin under my shoes, if they asked me to stand up from the wheelchair I would faint, and they immediately hospitalized me. Cardiologists, surgeons, pneumologists, and of course, gynecologists, came to see me and prescribed that I take an anticoagulant, with regular blood tests.
I must say, I trust my GP, after years of search, I finally found the right one. A drop into an ocean.
My life has been significally shortened by the lack of empathy, and the blunt incompetence of doctors (GP’s and specialists alike). I understand that medicine is not like math, it has to be taylored to a human being, and for this reason I say: YOU NEED TO FIND THE RIGHT DOCTOR. If you don’t like how they treat you, how they tell you things, or a medicine that they gave you is not solving the problem, first talk to them and try to sort it out. If it doesn’t work, for Mother Earth’s sake, ask a second opinion.
Doctors are just as valuable as teachers. No more and no less. We need doctors in rural parts of the country. They can be just like Dr. McCoy, just an old country doctor.

In the Netherlands, a female doctor opened a second opinion practice. There, people could consult with her for a full hour about their issue. Reaction? Other doctors were furious! I like her attitude and her empathy and caring for people it’s commendable. No doubt this was instilled in her through her parents who probably received it from their parents. The blatant murdering of the poor in NY during the current Covid19 crisis has permanently ended my trust of the medical profession. There were to many ICU nurses blowing the whistle about false Covid19 diagnoses with too many unnecessary intubations that resulted in premature death. The doctors demanded verbal DNR orders and did not inform the family. The patients were isolated, with no visitors allowed. After all, they might get infected. When she was 99 he kept her as confortable as possible But did not do anything more Then when my husband had stage 4 cancer he only did things to keep him pain free and as comfortable as possible knowing only a miracle from God could keep him alive he was 62 when he died. I recently hurt my ankle and went to emergency. It hurt but still had good colour in my foot, it was swollen but not much real pain, could easily move my toes etc… In fact everyone thought it was a bad sprain until they did xrays Not bad the emergency doctor described it as a stable fracture. A crack in the “plate”(?) that makes up part of the ankle. but everything in place with no other damage to anything around it elsewhere in my foot..I have been sent to a specialist whom I have not spoken to for more than 20 minutes total. 1 in person visit and 1 3 minute video visit. In that less than 20 minute total time he has mentioned “may need surgery 4 times. He gave me no information, no recommendations on exercises or anything in fact on the initial visit I felt like he couldn’t wait to get out and my questions stopped him at the door. I hoping he is just preparing for the possibility and that its so minor that he felt he didn’t need to “do” much but…
Almost right from my first visit to the hospital I was still able to move my ankle albeit with limited motion which is to be expected, over the last 2 weeks most of the swelling is gone, movement has increased and I can put my full weight on it being careful of placement. Waiting to see what the 3rd batch of xrays say. To be fair he has said that things were looking good so far but with 4 mentions of may require surgery it brings up that “fear” of is this going the route of needing it for profit. If he suggests surgery I’m going to go get a 2nd opinion for sure!
I see some glimmers of hope in the medical establishment. This young doctor is one of them. However, I do feel that the onus or at least the majority of it, is on the patient. We have the responsibility to keep ourselves healthy in most cases. Of course, if you get in a car wreck, it’s a different story. But, your lifestyle, what you eat and what you do everyday, are the main factors in keeping yourself healthy.

As a doctor, I find this talk a huge waste of time. People often forget doctors and their family are patients of other doctors as well. When people don’t see the whole picture, how will they be able to ask the right questions that leads of any possible solutions to the problems we are facing?
I lived and work in many countries and experienced many different medical systems. As a patient, the overrated doctor-patient relationship have never a problem for me. It is the kind of narrow minded thinking that patients and doctors who never traveled and experience different medical system and jurisdiction likes to fixate on.
In reality, the real trust a good medical system requires is the trust between patients and their government. Trust that their law makers are efficient at both spotting and solving problems in regulating medical services and pharmaceutical industries. In addition, the creation of an extra judicial independent expert body that proactively investigate incompetence and ethical malpractice with powers to sanction and revoke licence is in my mind, essential for any medical system.
It is only when you give up on the “big” societal picture, that “small” ideals like personal trust becomes more important.
Great credentials but something seems off. She finished her residency in emergency medicine at Harvard (B&W) in 2013 and has been giving these talks for 7 years. I don’t disagree with her issues but suspect she may have embellished her persecution. I have always used THE MEDICAL LETTER for drug info. I don’t see drug reps or accept their gifts. Hard to believe that Boston doctors are going after another doctor for “transparency.” Maybe there’s more to the story.

Could we analyse cultures in terms of #mutations and #selectionpressure?
If so, could we find traits that promote the survival of the culture, even at the expense of other forms of life?
For example, doctor-culture: could it be involved not only in professional standard-setting, but also in classic survival mechanisms, such as exclusion of competing ideas, conspiracy, etc?
Could doctor-culture display symptoms of #cult control? Of mental illness?
And I don’t mean to point a finger at any particular doctor…. My speculation is directed at doctor-culture, considered as a “living” organism.
It would be exciting if someone within the field could speak to this. There is a #phd thesis to do on this, irrespective of my own views.
An example title would be: Doctor-Culture and the Hypocratic Oath: where rubber meets the road.
The thesis could test whether doctors truly “do no harm” when they prescribe, as treatment, a medicine that is designed only to suppress symptoms. To what extent does this implicitly discourage investigation of the root cause? And can this behavior, if it exists, be linked to the survival of doctor-culture?

Yes! Yes Yes! I am a mental health practitioner in the UK. I have come across this secrecy and consultants with God complexes who think they run the the hospital when really the nurses and others are doing the bulk of the day to day work. They think they have a right to be rude to patients. I have come across doctors with great training and others, especially psychiatrists, with inadequate training so that they don’t know how to communicate in a human, connecting way, and are hidden behind such a wall that the patients might as well be seeing a robot. This can lead to misunderstandings and huge amounts of pain for the patients and for the psychiatrists who have walked into their own fear trap. Even though they may be in principle highly ethical they are unaware of themselves and their own deficits. Excellent TED talk. Thank you.

The romance with business medicine usa is bringing us to a heartbreak.
It was necessary to choose an oncologist 30 years ago. I have seen him through operations, chemo and radiation. It was understood at the beginning, that we would discuss treatments together but final decisions where mine. After being rear ended at a stop light by a motorist whom was not paying attention to 3 lanes of vehicles stopped at a stop light, I commenced to call my insurance agent upon arriving at my work place. She in turn notified the opposite party’s insurance. That same day, I took the remainder of the day off to see my physician. He recommended me to have an X-ray done, then an MRI and Cat Scan which revealed ruptured/herniated discs in my lower back #’s 1 and 5 area. After repeated visits due to pain and discomfort, problems sitting, standing and walking, my PCP had the gall to say to me, and I quote: “are you faking having those symptoms”? Hello, what kind of PCP would say that to his patient? That was 20 years ago today and am now still having lower back/buttock burning, needle like sensations and numbness on both front and back of both legs down to my feet area. Sometimes I tend to lose the ability to walk due to the lower back pain. The statue of limitations has expired so I am having to live like this the rest of my life. It’s sad that a physician as such is still in practice.
I like listening to her spoke. Besides she is a very beautiful lady. She didn’t let anyone or anything stand in her way or stop her. You are a strong and educated lady. Proud of you. ❤

I always keep in mind that people will rationalize whatever they do. So, I try to avoid needing a doctor, and when I do need one, I’m scared sh**less.
Great presentation. 100% on target. I wish I’d had her as my doctor a while back. A sloppy, lazy doctor can cause patients serious, permanent damage to their health.

Eat an oatmeal, a cup of Coffee or Green Tea and a banana with protein like Chicken…I guarantee you will have really good Mental Energy everyday for the rest of your life. Don’t forget to take Fish oil too, that’s important and other vitamins.

Goed, ik ga hier even in het nederlands op reageren. Deze dame is zaait twijfel over het functioneren van onze voedsel en waren autoriteit, waarvan ze overigens zelf het voorbeeld geeft dat ze weldegelijk eisen verscherpt op basis van nieuw onderzoek. Dit doet ze zuiver en alleen om miljoenen in te zamelen voor een apparaat wat absoluut niet kan functioneren zoals zij het voorstelt.
Dat ze maar fijn haar “wonderprodukt” aan volkomen paranoide amerikanen gaat verkopen.. Oprotten trut!
