How to treat Groin Injuries in Runners

Many athletes with groin pain have tried prolonged rest and various treatment regimens and received differing opinions regarding the cause of their pain. The rehabilitation specialist is often given a nonspecific referral of “groin pain” or “anterior hip pain,” the cause of which can be as simple as a tight iliopsoas (which may require stretching) or as complex as athletic pubalgia.Hip and groin pain in young athletes can also be overcome by heat compression methods.
It applies for the same conditions as that in the case of cold compression, except that it has a different mechanism of action. Application of heat to the injured area aids in opening up the blood vessels for an adequate blood flow towards the wound.Muscle and tendon injuries and osteitis pubis will often respond to appropriate treatment, although the time course may be prolonged.
Surgery is frequently indicated for athletic pubalgia or sports hernia. It is critical to rule out a femoral neck stress fracture in running athletes with groin pain, no palpable tenderness, and a positive hop test.Interest of a meeting about groin pain •Common injurie in sports medicine 5% professional sports players (Anderson and al. Am J Sport Med 2001) 10 à 18% / year soccer players (Morelli and all. Prim Care Clin Off Pract 2005) •Poor medical literature 104 papers Medline literature between 1950 and 2008.
Groin injuries comprise 2 to 5 percent of all sports injuries. Early diagnosis and proper treatment are important to prevent these injuries from becoming chronic and potenYou will probably have had massage, strengthening, physiotherapy, and maybe injections to keep playing. Steroid injections to the adductor muscles, pubic bone and symphysis are commonly used, but do not ‘cure’ or fix, neither does rest – it is not treatment.
Now lets look at the forces. The available evidence from the randomized trials is insufficient to advise on any specific conservative modality for treating exercise-related groin pain. While still low quality, the best evidence is from one trial which found that exercise therapy (strengthening of hip and abdominal muscles) in athletes improves short-term outcomes (based primarily on pain measures) and return to sports. There are many types of groin injury, some of which can occur together. In fact, as many as 90 percent of athletes experiencing groin pain have multiple types of groin injuries simultaneously.
1. Common types of groin injury in athletes include, but are not limited to: Adductor strain (groin.Immediately after injury, the goal of treatment for a groin strain is to reduce pain and swelling. The first few days of treatment follow the protocol for.
Groin pain in athletes/recreational athletes is a very common complaint. The nature of the pain can vary hugely depending on the cause or mechanism of injury. For example, some will report pain with coughing and sneezing whereas others might only get pain after high intensity training.Groin injuries in sport are very common and in football they are among the most common and most time-consuming injuries.
These injuries are treated very differently around the world. There is no consensus in the literature regarding definitions, examination methods, diagnosis or treatment and in general the level of evidence is very low.Context: Sportsmen’s groin, also called sports hernia and Gilmore groin, is one of the most frequent sports injuries in athletes and may place an athletic career at risk.It presents with acute or chronic groin pain exacerbated with physical activity. So far, there is little consensus regarding pathogenesis, diagnostic criteria, or treatment.
Pubalgia is a common injury in the groin area. It’s also known as a hockey hernia, Gilmore’s Groin, or a sports hernia. Despite its many names, pubalgia isn’t a.Lifestyle Treatment Options. Some causes of groin pain require simple strategies that you can often do at home. For instance, for a groin strain from a sports injury, your doctor will likely recommend rest, icing the injured area, and wrapping the upper thigh with an elastic compression wrap to decrease pain.
Usually PRP is used as a adjunct or secondary treatment to surgery or in this case Prolotherapy injections. A 2015 case report published in The Journal of the Canadian Chiropractic Association ( 10) offered the case of a hockey player with recurrent groin pain.

Hi that’s a very informative video, I am a runner, cyclist and regular with my weight training. I feel acute pain in lower abdomen, specially after running and unable to do my lower abs as well. I have my epidomic kidney just above the pelvis area. Hi! After having intense runners knee in Sept 2018, I started going back to running around March 2019 and adopted the front foot landing technique. Since March I’ve had an achilles tendon injury and another slight knee injury and now pain on the inside of my groin (abductor). It’s just so frustrating because I want to get back into my full workout routine/fittest self (5-6 days a week training). Do you have any tips to prevent the injuries to keep building up? The video was also really helpful! Kind regards!
Hi! Thanks for your videos, have been very helpful so far!!
I started having pain between my inguinal ligament and hip flexor during a running session. Carried for 2/3 trainings and pain ended up being worst so I stopped running. Have Been almost 4 weeks, X rays show nothing, 3D picture coming next week. Pain occurs in that part and upper the knee when putting tension on that part for example raising the leg with 45 degrees or more.
Any suggestion what could be the root cause? Docs and phisyo running out of ideas:(.
Soo many thanks in advance and keep the good work!!
Miguel

So I’ve had a recurring pain in my groin for a while now, and I reckon it started because of my poor squat mobility. There’s pain high in my front/inner thigh. And when I squeeze my legs together it pops in that area of both of my hips, but it’s a painful pop in the right one. Then it’s a bit less painful when I squeeze my legs together. Also on the right side, I noticed that when I tried to throw a side kick with my right leg, I came away with outer hip pain. Almost like the outer hip was too weak. And in general, I often feel like there’s a difference between my hips in terms of stability. Very strange stuff. Any thoughts?
Best video I have found. I pulled my groin training for the NYC Half marathon its been 4m without running but I still feel it. How longer is the average wait to stop feeling pain? Summer is coming and I am getting anxious with the pandemic of not being able to go out and enjoy the weather. Please help..
Very useful video I have pain in my groin region when I starts to run in it hurts more and when I sneeze it causes slight pain in my groin region I can’t find the reason for this pain
So plz say what must bee done to prevent this injury
Plz say
I’m thinking I might have tendinitis in my adductors but recently I’ve been having a ton of pain in my deep adductors and it eventually spreads to like my hip flexor and then if it gets bad enough my whole hip socket. Been resting on it for 2 weeks and it’s slowly starting to feel better but is there anything else
I can do?
Hi, Im a soccer player and I was diagnosed with osteitis pubis, so can you help me with that rehab excercises including strength excercises for core, glutes and hips. Should I make a physiotherapy? Should I take a medicaments?, (and if its yes, what type of medicaments I need?)How long should I recover?
Football is just around the corner and I need to get in shape but this is a problem I’ve been having the past two days. It hurts to sprint so I jog. Should I stop or keep jogging it out?
I’m almost 100% I have the tendon one.. What excersies should I do to recover faster?
Thank you for your help so far and there is a delay in me getting to see a pain management specialist to get this torn ligament in my groin that is by the way against my Colon and every time I pass a lot of wind or push myself to go to the toilet I get pain with it but what I would like to know if you can help me again here and that is, is there any chance of this torn ligament ever healing on its own or do I have to have injections and is there anything I can take to get it to heal on my own.
I had a ultrasound on the inguinal canal but it only revealed some muscle bulging
This theory is useless you have to demonstraight some exercise for reciver groin pain
Hi, may be i have issue with hip flexor.. As when i move my leg sideways while standing it makes sounds as something getting brush inside….. It hurts while running. Please suggest what to do
Great video. On feb 13, I over extended my right leg, in gym, and I heard a loud pop sound above my genitalia and felt immediate pain by my upper right thigh. It’s important to note that made the mistake of not stretching first, and I’m not a flexible person. For the first month, when I’d try to light jog (even very short distances) I’d feel a very sharp pain and have to stop. I never had much problem walking, unless it was fast.
Now, 3 months later I don’t feel much pain and really none when at rest. I initially couldn’t do leg raises on my right leg now I can do just as many on my right leg as my left leg. Also, when I touch the area I feel no pain, which I think is good. However, When I sit or lay down in certain positions I occasionally feel minor/negligible pain so I know I’m not 100% but I see improvement. I don’t know the time frame for this injury. Would you suggest an MRI or do you think that progress is ok? And should I be doing complete rest or relative rest where I work some strengthening exercises? Thank you
I have groin pain for 10 months, I fell in football and i heard a sound in my groin. Please what should I do?
Hi I saw your video and it is very informative and useful.
I want ask some questions, could you answer of those questions?
Case history:
I got groin injury 1.5 year ago in my left leg while sprinting and I consulted with the orthopedic surgeon specialised in sports injury but it didn’t work out for me..I still feel some stiffness in my injured part and I can’t sit with folded legs.
after 1.5 year I again got same injury in my right leg while doing some yoga exercises, so could you tell me
1)What is the first aid for groin injury?
2) after how many days of injury rehab exercises should start?
3) why I am getting groin injury frequently and repeatedly?
I’ve had this groin injury for several years. It comes and goes. I think it originally came from playing soccer, but I can’t remember anylonger. I can feel it sometimes even when I haven’t been exercising.
thank you so much for this nice video, am a physiotherapist, please direct me where l can go online or as you said to skype with you guys because i have somethings to learn from you

Why are oranges Orange. Is that a coencedence like apples aren’t called reds. I think someone was just being lazy. Your thoughts on this would be much appreciated thanks
i have seen your video,it`s very helpful for me. but answer me to about:how much uric acid effected at groin area?
What would the type of pain be from me sprinting and my groin hurting.
I suffer from groin pain and pelvic floor pain, I am male, I find it very hard to get treatment because most therapists don’t want to touch the groin area because it’s too close to my penis. And pelvic floor is close to my anus and testicles so no one wants to give me treatment I am suffering
Hi,i’ve been running for a a few weeks now,but I have this intence pain in my right hip/groin. It is just next to my right hip and it runs down to just below my stomach. I started feeling a pain in the first week and currently it is getting worse. When I press/add force to my hip bone,it pains and any tipe of movement after my run is too sore. I can only run every 2de day as it hurts to much to run every day. I usualy start of fine and halfway through my run it hurts so that I have to stop and rest. What must I do?
Thanks! Helps out a lot, even though it’s very hard not to start full on running again.
What a great video, thank you! Been experiencing slight pain in my inner thigh on my last few runs and this has clearly explained what’s going on and what I need to do! Subscribed and I’m sure I’ll be back.
From the picture in the video I can conclude that it’s most likely my hip flexors that hurt when I run, it doesn’t hurt in the start but when I reach 4-5km it begins to hurt. I’ve been running for almost a year and this only just started recently, not sure why. Still not sure what to do, I’ll try and keep more attention to how I run, but it’s just weird that it’s only starting to hurt when I run now but didn’t hurt when I started running about a year ago. I’m only 20 so it shouldn’t be because of old age yet I hope
I been having a groin pain for a long time. I rested so about a month it felt good so I started running for about a mouth and than it came back please help really I’m in need of help
Hi doctor

I have groin pain only after running and during stretching. Not during running. Is there an explanation for this?
Glangwilli hospital has been on the phone to me to say that because the last injection I had in my groin for the torn ligament I got there did not work I got to now see a pain management specialist to have a stronger injection under anaesthetic and they will let me know when, All I can say is that hopefully I will get it done before Xmas.
Thank you this video helped me with everything you said!! I’m slowly getting into running againa dnwhen it feels like its creeping up with pain I walk also. But I’ve been running for 12 years now so I listen when my body says stop. Thanks guys ♡♡
I have athletic pubalgia or sports hernia. I’m miserable. Its been over a year in half. Can afford a sports medicine doctor. Can you help? How do I rehabilitate my groin and lower abdominal muscle on both sides?
I started to have pain and swelling in my grown weeks ago and I was sure it was a hernia but I seen a consultant yesterday who examined me and he seen it was a Inguinal muscle tear and he gave me a painful steroid injection that he said will take a few days to set in and work and get rid of the pain and swelling and will last for 3 months and then go back to see him, well as yet the pain and swelling has not gone but hopefully it will in the next few days, what do you think.
Hey can we Skype or talk on the phone. I just need professional evidence during this time, many offices are closed.

What is the difference between ostitis pubis and groin injury
What about the lack of stability in the joint connection I mean where the femur goes into the socket in the hip. to keep that area tight and not slippery and not jumping in and out out of the socket seems potentially able to solve a lot of problems what do you think about that?
Hi,
I had a groin injury a while back. I felt a lot of pain. Then it stopped and I started playing soccer again. But right now it just comes and goes. How can I stop it permanently
Thanks for this informative video. You hit on the main thing that confuses me which is that my hip/groin pain hurts at the start of the run, then dulls or goes away as the run progresses. Afterwards, it feels ok, but within a day or so, the pain comes back. I’m feeling it in two places on my right leg: in the area of the abductor and outside of my hip and it’s usually when loading only that leg such as going up/down stairs, running or doing a single-leg squat-I feel no pain when squatting with both legs. Is it possible that one injury can be felt in two places, or might I have two separate injuries? I do plan to see a physio in the next week or so.
Hi and thanks for this well informative video! Exactly one year ago during a one month trip that was include lots of walking…on the last week of the trip I started to feel very deep pain in my inner thigh until a point I couldn’t walk anymore. so I was resting and it went back to “normal” and kinda disappear. 3 month after I started running, during the running everything was alright but after around 3 more months I started to feel this pain again but this time it was combined with a deep “stab” feeling on the adductor muscle and since than this pain is exist. I was diagnosed by a hip doctor with “adductor tendinitis” who recommend on physio treatment including deep friction. So I went to a sports physio who did deep friction combined with several electrodes treatments around my hip sadly to say that after a 2 months of physio (once a week) the pain is still there (comes and go). Than quarantine started ( two months ago) and had to stop the physio treatments.
I have to say that I cycle, walk and this pain isn’t giving me a hard time it is just keep on being there and each time I might run a bit, squat, lift the pain is hard enough so I cant train for few days. the pain is much painful when I turn the hip up and against its side.
I am at a point that I am thinking if going back to the doctor to ask for better answers coz I really don’t know what I should do so this annoying pain will heal that also effect my groin a lot.
I will appreciate your opinion a lot. Thanks!!!
P.s do you think there might be connection to lower back bulging Disc? do you think chiropractic treatment will help?
<3
I can walk and even run but not really that fast…when I sprint it kinda hurts in my right groin what do I do???
I forgot to add Yes it is a torn ligament and they have used a diagnostic ultrasound or MRI imaging to confirm it is in fact a tendon/ligament issue I am now taking different vitamins as well as Collagen powder and do you think this will help heal it.
Hi. I have pain on the inner thigh near the thigh and the lower abdominal aftrr playing rugby. It makes me hard to get up while lyinh down and when i cough it is really painfull and the pain is sharp. Really need your help and advice
Me again and 16 days on and the steroid injection still not worked I been on to the hospital and they will let the consultant who injected me know and take it from there.
Am not a runner however after birth of my now 3 year old and seeing a therapist am sad to say my groin area is still bothering me. Sometimes when i walk my groin is unbearable. Any suggestions
I have the adductor tendonopathy—the docs and PTs I have seen have not diagnosed it properly—this video described me perfectly.
Hello Madam, Thanks for the reply.
The issue with me started with pain on the right adductor region which comes and goes while i play. I checked with a Dr when this came frequently after one game of football. He checked my scrotum and even made a scrotum scan, which revealed that i had a grade 1 varicocele. I then consulted a surgeon and he gave me medicine for a month and added to give more support for scrotum. Now the pain comes back when i play and it is spread around the pubic groin area, goes away in 2-3 days of rest. it comes back with any kind of physical activity. i seriously doubt that varicoceles are not the cause for my pain. 2 months of lockdown in the country has resulted in sudden decrease of physical activity and all this came up when i started again.
It has given me great distress over the past few weeks as treatment for a varicocele is quite fearful and i am an enthusiast, not being able to play is like suicide for me. Also planning on doing some things in my life which requires high intense physical work. Can you suggest me something, is it just the varicoceles or have i injured anything else. Really waiting for your reply.
@Sports Injury Physio Thanks for the information. Around 2 months ago I pulled my groin (the adductor muscle), after some time I was able to sprint and play football but I guess my muscles weren’t strong enough and I pulled it again like 15 days ago. Now when I do my strengthening exercises I feel the pain but I’m able to jog slowly. So what do u think would be the minimum time before I start going out and play football because u know once the quarantine is over I might push myself and reinjure
I have burning pain at my pubic bone, this is new I fell recently. But I’ve had problems in my inner thing and testicle on and off since I injured myself 3 or 4 years ago backpacking in Sequoia Tablelands where I had to step across huge blocks of stone over and over jumping from rock to rock. Walking is fine, sitting hurts in pubic bone if I lean forward to see the computer or if I cross my right leg over the left, I get a sharp burning pain. thanks for the video.
i have pain in the junction of the groin. I dont feel pain at the inner thighs or anywhere but at the upper joint and by seeing your video, I believe its adductor te,ndinopathy. I play football on weekends and i do gymming on weekdays and I make sure to stretch after workout wherein I also focus on groin and other lower body related stretches. Although I feel pain at the start of playing football, as the game progresses I’m relatively OK with it. while warming up to the game it hurts while sprinting. And after the game and in the next days, it keeps hurting slightly while I make day to day movements. What do you feel might be the cause and how should I go about treating this. Should I stop playing football but that would be very hard to do for me. I love playing football. I will be eagerly waiting for your reply. Thanks in advance.

We can’t hear you. The volume isnt working. I have had a problem in my lower abdominal wall and groin area for almost a year. I have rested and quit exercising for 3 1/2 months and it doesn’t seem to get better. I need some advice.
Fellow Physio here Mid 20s Male Isometric hip flexors, adductors etc are fine not pain producing -ve labrum involvement, -ve squeeze test. Main signs for me is single leg standing/squatting/weight bearing it is not painful to complete but once the weight moves back off the unaffected side my deep groin pain is replicated. Any thoughts?
Great content! i really like you videos and explanation methods.
I’ve been getting physio for one year now for groin pain. I’ve still got it and while I’ve seen progress it’s very slow. It’s very frustrating.
Hi,
Last Sunday I was playing soccer and I was late so I did not stretch. I was totally fine after the football I did not feel any pain I just felt a bit stiff. When I woke up the next morning (Monday Morning) I started feeling very painful Groin pain. At the time I was struggling walking around etc… So 3 days later I can walk fine but it is impossible for me to run! Do you have any advice for me and how long should I wait until I start playing soccer again?
Thanks
I went into the middle splits and my adductor got sprained and cheer tryouts are in a few days. I still have the pain, this is the day after and I’m just nervous it’ll last long.:(
I had a groin pull from a slip fall in a cafe where there was water on the ground. This happened in 2016 but I still have pain. The pain runs from my groin, inner thigh down the leg. In the last 6 months I have been experiencing pain and numbness in toes of left foot and side and top of foot. There is weakness and instability of the foot. Do you think this is related to my groin injury 4 years ago? Nerve conduction tests requested by Neurologist showed no nerve problem.
Please help me?? I can’t find what’s wrong?
Yesterday i injured my groin on the inside thigh region area. I was doing a 10m starts on the grass when at the 10m meet i felt a pop in that area and i slowed down. im stil able to walk but i can feel pain and discomfort in this area. Ive been icing and taking ibuprofen for it and i dont feel it as much. Is there anything else i should be doing?
