I'm helping coordinate our cohorting service for floor level and intermediate care level patients at my tertiary care center. Fortunately our number of rule out patients has been low the last few days, but policy changed and we are no longer limiting testing to those with non-community exposures and smartly are testing anyone with worrisome symptoms in the ER/hospital.
With that, our census will almsot certainly explode. I'd like to have the best standardized documentation templated because time wil become precious soon, and anything I can save my providers from documentation can be used in patient care.
Those of you that are ahead of me and getting into a groove, do you have a problem based history, physical or A&P standardized for COVID that you find useful, or maybe even essential to hit all the important points and get the right information across to cross covering teammates?
I'll be making my own, but with all of the experience that's out there and that might already be seeing common complications or issues, any input shaping this would be greatly appreciated!
